The Client Forms are used to create supplemental digital forms to the Policies and client/pet profiles (Client/Pet Fields) - if you're unsure of what to use see the "How do I add my forms to the system?" FAQ. They can be required by your clients before they request services.

  • Create the form by using Petcare > Client Forms. Here you set the form name, where the notifications are sent and whether it’s required per-pet or per-client
  • After you create the form you’ll then use the “Manage Fields” button to add and reorder the questions that will be on the form
  • Once the form is set to Active it can be filled out. You will see submitted forms by going to the Communication > Submissions Admin page.
